•  Two standard cleanings (prophylaxis) per year
•  Complimentary fluoride cleaning
•  Two emergency visits with X-rays per year

How do I enroll?


1.  A one-time fee of $395 covers your cost of our ADP for a benefit year.
2.  A monthly membership is also available.
3.  Speak with one of our treatment coordinators to be enrolled in the program
4.  Once added to your chart, team members will be aware of your enrollment and you start enjoying the perks immediately!
